Certificate Reprint

Effective March 23, 2007, only certificants holding a CI-CPT certification will be required to renew their certification. All other Cooper Institute certificates will become open-ended with no expiration date, and will not require renewal.

You may now request a reprint of your certificate holding no expiration. Please fill out this form and send it along with a copy of your original certificate. This form does not apply to the CI-CPT Certification. Make sure you retain the originals and a copy of this form for your records.
